Guardian Nomination for One Parent
Just answer some questions, download, and sign.
Designed for one parent of one or more children under the age of 18 years old. Does not include any other documents.
Regardless of whether you, the parent, are married, single, partnered, etc - this form is designed for any one parent who wants to nominate guardians for your child or children who are under 18 years of age. If you and the other parent want to nominate the same guardians, check out our Guardian Nomination for Two Parents.
What this covers:
You have the right to choose who would care for your kids in an emergency. Make sure your voice is heard. If you think it’s hard to choose someone, think how hard it would be for a judge who never met you to make the best choice.
How it works
- Choose a guardian for your child/children in case you are unable to care for them yourself for an extended period of time.
- If your preferred guardian is not in California, it’s even more important that you put your wishes in writing.
Included: California Guardian Nomination form for one parent.
Not included: Will, trust, inheritance planning. Does not handle your assets or your own personal care, just your kids.
Investment — $75
A better way to DIY — with options from a lawyer
This DIY option was created by a California wills, trusts, and probate lawyer who has spent over 15 years drafting documents, administering trusts, and walking people through probate court proceedings.
It’s designed to be comprehensive, not bare minimum. You’re guided through the questions that actually matter, and your documents are generated based on your answers — not left blank for you to figure out on your own.
Our goal is to make it straightforward to use a lawyer-designed solution, without the confusion, errors, and guesswork that come with generic DIY templates.
